Certainly instructors have the most routine contact with students so it makes sense that they are likely the ones to identify problems. What's the mechanism to inform program advisors? Does the instructor have any responsibility for what happens after that?
Admissions is concerned about making their numbers, but most admissions people are also concerned about giving the proper information. What can you do to help get everyone on the same page?
Sounds like you have the right players at the table. How often do you do these reviews? Who takes the lead in the process?

You might want to consider more frequent reviews of students. A lot can happen in half a term. Who's involved with the reviews that you perform? What aspects of the student experience do you evaluate? Grades? Attendance? Non-academic issues?

Good - and easily done - action to assure that everyone is properly informed about services. It's easy to take things for granted and assume that everyone knows what's available.
Is the orientation mandatory? Is it offered at multiple times? What is covered and how do you know if it's achieves your objectives?

How do you make extra tutorials available - online, in the classroom, in a tutoring center? What's the level of participation? Are your students able to find time to take advantage of this extra help?
