I do not believe in giving extra credit. I consider this grade inflation and have seen this practice go completely out of control. I do believe in identifying students who did not do well on a graded assignment and bringing them into my office to discuss what they can do better in the future.

I will only give extra credit when the class as a whole doesn't average at least 70% on a test. Then I come up with an additional learning activity that based on what type of effort they put forth in the activity can raise that low test grade up to 1 grade level higher. This allows the student to have an opportunity to work on the topic again and reinforce some things that didn't go well the first time around.
I give extra credit with homework and on Quizes. It is usually either a review of something they covered earlier or a brain teaser to make them think outside the box. Students appreciate the chance to make up for something they may have gotten wrong earlier. I have found that it is not the students who are having trouble who take advantage of the extra credit. It is the obcessive students who really want to do well and the thought of missing even one problem upsets them.
I also use the online help as extra credit. Our textbook provides guided practices as well as taped lectures of things I have covered in class. They can use these for review then take an online quiz I made up for extra credit. This encourages them to use this tool.
