I do notice those little signs from the students such as when they have that lost look and are frantically looking through the pages of the book to find in the text what you are discussing. I will usually call on that student and present them with a question relating to the text subject matter. If they are still lost I will point out to the student where to look so they can get caught up with the current discussion.
