I believe its not just students who have trouble taking tests.

Test taking and test creating can be quite tasking. This cuts across the spectrum from the course developer to the student and include SMEs used to validate the test as an outside review source.

Some of it is language, how the test is created, verbiage used, and type of exam.

There is also the external factors affecting testing: lack of sleep, jobs, family, fear of testing, and class room dynamics.

When looking at results there are issues with testing, was it too easy, all 100's, to hard, barely 70 by the group. Additionally, did the instructor ensure the seeds of information were planted and retained by the student to ensure they can recall the info for the test.

In some ways the use of PPT has become the crutch in instruction and in testing. Something which also needs to be 'checked' in the education process.
