How do you effectively instill collaboration in student teams?
I have students count off and then break into groups. Invariably, someone takes a lead and others follow. However, there is usually at least one student who doesn't want to participate fully.How can I encourage all students to participate fully in team work?

this is a great way to evaluate/assess the work of the student groups & to ensure that you are connected to what's happening.

For large projects, I always include a peer eval. If it is a small in class group project, I walk around and sit with various groups. Mostly I just listen but I have also guided them on the right track or answered questions. They see that I am connected to the activity, not just sitting doing something else with them left to their own devices.

I agree that peer evaluation is a helpful tool to use in group work and group presentations. It is important, however, to set parameters and teach the students first how to give and take constructive criticism. Otherwise the peer evaluation can turn into conflict if not handled properly.

I have students break off into groups too. I make sure each person in the group has a roll; note taker, leader, project creator, etc.. When they present everyone has to speak.
i usually let my class interesting and make it fun for them at the same time it stimulates the students and their learning process is enhanced

the individual grading is a key idea to make sure that no one is hiding behind the work of others.

I find that asking the inactive student questions related to the activity usually sparks some activity. Also I make sure that they are being graded individually, even though they are working as part of a group.

this is always difficult; one suggestion would be to incorporate a peer grade so they have to participate or be downgraded by peers.
