Keep a running track of the questions received via email/phone and asked during the chats. This provides a listing of the major areas which needs more information or possible revision.

Like the others, I check-in with students during live chats, asking if they are encountering problems. Sometimes issues don't reveal themselves until I have looked at the students' first assignments. When I notice problems occurring repeatedly in student assignments, then I prepare an intervention. Usually, this is an additional tutorial or practice exercise added to the discussion board.

I love the immediate feedback so we can make changes before we do it again. Formative assessment helps students and instructors understand what they don't understand. ;-)
Like, Yvette, I obtain formative course feedback during weekly chat sessions. I ask students about the assignment, their understanding of it, and based on their responses I built resources to address any changes that need to be made, whether it is further clarification in the chat, announcements/Q+As or web resources.
I also pay attention to e-mail concerns or confusion that point to the need for formative evaluation of a particular unit or concept.
I do a lot of informal formative course evaluations, most often along with the weekly synchronous meetings, such as chat. I might ask for immediate feedback of the session to see if it was helpful. Also during chat, I might use that time where I can get immediate feedback to assess how the course is going for them or what issues or concerns they have.
Also, I'll get informal formative feedback in the discussion or via email. Whenever I have interaction with students on a one-on-one or small group basis, I take that time to get feedback on the course. It is usually very enlightening.
