Is there a specific formal document that must be used when a request is given to a school or is medical documentation good enough to use as a request?

Typically the rule of thumb is, if you can determine what the student's functional limitations are, how they impact the student in an educational environment and how to reasonably accommodate, the documentation is sufficient.
