A change from an A to an F certainly is dramatic and should be a cause for review, but how about a student who has been getting B's and C's? Is an F a call to action for them?

I find that comparing a students attitude first day of class with each class interactions will alert a instructor that something is wrong. For example if a student start out making A's on quizzes then start making F's, its a sign that they are no longer engaged in the class. I try to find time to speak with that student in private. Some times I'm successful at bringing that students focus back to the objectives of the class and I imediately see an improvement in the quiz scores.

Correct, willing to help them & giving them real options that will work is the key

Definitely keep an eye and an ear on your students. You will most of the time be able to tell if they are having issues just by their body language and demeanor. Be willing to help and guide them as needed...

I totaly agree with Mr. Beach, its amazing what a little communication and understanding will get you.

I agree Mr. Troy a student will show a change in behavier and it maybe small sometimes but its a good sign they are having troubles here or out of school.
