how often should you be evaluated & by whom

There are two components to this question. One is evaluation by students. This normally comes at the end of the course using a standardized college form. The college administration generally collects and review these results. Another form of evaluation is student feedback on a more informal basis. I give my students 3X5 cards and have them respond to questions I ask them about assignments once they have completed them. This way I keep my finger on the pulse of the class.
The other form of evaluation is by college officials like the Dean of Instruction or Faculty Supervisor. They sit in and observe faculty while taking notes. Later they have a one on one session reviewing their findings and make recommendations for growth. For new instructors these generally occur at the beginning, middle and end of a course. For experienced faculty these are generally done once in a course toward the end.
