I prefer test banks as well but I make sure to note the page number of the question found in the text. When a student challenges the question I remind him/her of the page that we covered the question. Sometimes a student may be absent on the day the material was given but the student is always informed to review the material covered afterwards. I also use fill in questions for quizzes so information is retained by word association.
It depends on what I am testing. For definations I normall use matching or multiple choice. For working out large problems such as an income statement they have to prepare the statements from information provided.
I pull test questions from a test bank on the chapters we are covering and try to look at the questions from the students' perceptive when selecting the questions. I review my tests every term to determine if I covered all the key points. Most of the questions I pull from the test bank are multiple or multiple multiple choice questions, since that is the structure of my profession's national boards.
I generally make up my own exams, and I tend to revise them term by term. Each class/term is different and I try to gear them towards each class. I do however, stick to the terms/objectives of the syllabus.

It varies for me as well. Generally for composition classes their writing assignments are their test grades. For Grammar, Multiple Choice and application work best for me.

If I want to test facts, then the fill in the blank type of question.
If I want to check analytical abilities, then multiple choice.
If I want to give a lot of freedom and test artistic abilities, then an essay.
Hi, I also dislike true and false and avoid that type of questioning.I prefer to use matching for vocabulary ie. medical terminology. multiple choice questions for physilogy. Most of the time I use a combination of multiple choice, fill in the blank, or matching questions on a test. I also consider the number questions and time constraints of the class.

Although I don't like True/False questions, I do like a Modified version of True/False. The students have to correct the sentence in order to make a True statement.
I agree, it really depends on what kind of subject you are triying to teach. For a lab setting you have to have the hands on experience in order for them to pick it up.
Based upon the course objectives and the course being offerred, there are a multitude of questions that can be used. In a theory course, I would use essay questions so that the student can share their ideas and make a connection between the content and personal experience. I would use a multiple choice testing format for a course in prevention, becaux=se there is usually a "best" answer to pick and this will allow me to "know" where the student is in reagard to material. For a clinical test, I would use a skill application method.

Hi Jose - Good rationale for assessment choices! Susan
testing is all based on what subject is being taught.
pratical application works for skills etc.
I agree it all depends on the type of class or lesson that is being taught.
things like handwashing and gloves require praticals as opposed to medical term where multiple choice and fill-in will challenge the student
By far the use of a essay based format needs to be used more since language and writing skills continue to be lost with the advent of texting...
This section has helped me tremendously in the reasons to use the different testing formats. Before I knew what worked best in determining what the student had learned if anything but now I understand the reasoning behind the different types of tests and why a particular format should be used over another.
