Talking with the students as a whole and getting them comfortable with you gives them a sense of trust to come and talk to you about their problems if they exsist.
Actually we are not supposed to ask if anyone has learning disabilties. But I do have an open door policy and tell all students if they have any problems, concerns, etc. to come see me - that communication is the key to success.

I agree that it is difficult to ask students about sensitive issues - however - it is OK to announce in class if a student has any learning challenges for which they need assistance, please stop by your office.
Just letting them know you are there and willing to help will give them permission to ask for help.

Unfortunately with pivacy acts making it unallowable to ask students if they have problems, it becomes difficult to identify and to help.
