Many times in dealing with students I have found that with a little detective work before meeting with them, you can discover what the "real" issue or issues are. There is no such thing as asking the wrong question, it actually is asking the question the wrong way.

I have found that being straight forward with students who may have issues rather than circumventing the issue works better for the student. They realize that you are truly interested in helping them with the problem because you get the explanation first hand from them.

I like the way you are pealing back the layers to find out what the "real" issues are so you can address them. Often times instructors get caught in trying to respond to perceived issues rather than the real issues and when this happens the problems just continue to grow.
