You have a student with manic tendencies that frequently participates in your lecture
State prior to lecture there will be time for questions after the section has been completed.
Involve all students and explain to the manic student how it's important to obtain different points of view from others to appreciate the many experiences we can learn from.

I have had dealings with these type of students as well, and I do exactly as you do. They tend to accept the directive of letting others have an opportunity.

I have one student in this category. He comes up with very valid questions, but he has many questions! Throughout a class day, once I've answered 2 or 3 of his questions, I calmly state that I need to give others an opportunity to ask questions as well. Simple, straightforward, and the student responds immediately and positively to my response.

Involve all students. I make it a point to call on every student as often as possible.

Always answer their first question, then ask the rest of the class if they have any questions also. At this point announce that you will be stopping periodically to ask for imput. However, you will make it involuntary and call upon the students specifically. This is a good practice to use.
