I have found that instructors need to be incredibly adaptable to the needs of challenging students - I encounter both legitimately challenged and attitude challenged students every class, and there is no one easy fix. I try to make them feel as comfortable as possible, while at the same time letting them know that I have the same expectations for them as for everyone else in the class. One-on-one time scheduled with each student has been helpful for me - I take 5 minutes every hour or so to "check-in", and discuss the days's progress at the end of class. We then create a plan for the next day, and so on.
