Managing the e-learner environment involves consistent review of assignments, constant communication, and student outreach. This way you are more aware of what your students need and how to best capture their attention.
With each new class you teach, we also go through a learning curve just like the students. We abide by the syllabus and instruct the material in the order that it is given. But after we have instructed the class a few times we can begin to loosen up and expand on area’s that have given students trouble in previous classes and continue with what works for the other areas. I have also found at times adding a little humor (within reason) in the live chats seems to loosen up the students responses and makes it more enjoyable for everyone.

How do the department head grade the course? Do they have access to all of the online courses and log in and evaluate the instructor's interactions in the course? The design of the course? The quality of the interaction What measures are they using to determine an instructor's effectiveness, the same ones as a traditional course?

Absolutely! You get evaluated in resident courses from both students as well as Department Heads. The same should go for e-Learning as well.
There are some instructors that don't seem to have enough time to follow this format. I believe e-Class observations are critical to maintain quality.
