The purpose of a Form PS-042R is to summarize and itemize any changes that are being proposed. This form insures all changes are documented and easily identified for both TWC and the submitting school.

Bobbi,
You are correct regarding the basic purpose of the Summary of Changes PS-042R and the results of not submitting it to CSC. I would suggest that you also use this form on a regular basis to monitor changes to your catalog. Keeping a catalog on your desk where you make notes of changes when they occur will help you to make sure that you submit your changes on a timely basis.
Gerald Parr
It is to notify the TWC of changes or revisions and allow them to authorize the changes. Otherwise, any changes made to a catalog (for example) would be deemed unauthorized during a site visit.
Absolutely right! You get a new catalog or new addendum, be sure to send TWC a copy. Don't forget the PS-042R and a list of changes.

If a school introduces homework assignments into an existing program, do the institution need to complete a Program Revision App? The clock hours and curriculum remain the same.
That is one of the reasons. The main reason for the PS-042R is to ensure that this is an official change and not an unauthorized change. You notice that the form must be notarized.
