functional limitations

I suppose in some cases it's very difficult to determine an individual's functional limitations even if one has personal interaction with them. It
seems that the institution needs the help of experts on the particular field

Actually, the best person to help determine the student's functional limitations is the student himself! Self-report is generally accepted and promoted as the logical way to establish such things. BUT... there should be someone at the institutional level who is prepared to take that self-report (and/or whatever formal documentation is brought from outside) and turn it into an active "prescription" for accommodation. Knowing what the limitations are and knowing how to work around them are two different things, and I don't think instructors should be asked to take that responsibility.
