Students that have work from previous students
I have learned to be on the lookout for this because they will use work from previous students. What I've done is change the assignment a bit so that whatever they got will not work for them.
A simple way to utilize the same test, to make sure the exam is covering the material that has been discussed is to scramle the test questions. This way the student cannot come back and say they were given a more difficult exam.

I really like the idea of having 4 copies of each exam. This does make it real tough to cheat and get a copy of the exam. I simply let my students know that they are being watched, and I watch them like a hawk. I do not let students keep any graded papers.

Ideally I like to have 4 copies of each exam (excessive and difficult to accomplish), so to get a copy of THIS exam (without knowing which exam is was) you would need to contact a student from 2 years previous.
I have also told the students that the first itme I know of tests being passed out- no more exams will be returned to their care, and I will collect everything.
Granted, my class size is small currenlty, but using fill in the blank and identification, helps to decrease cheating since the ID portions is not avaialbel outside the classroom!

Students will get their hands on the work of other students. At my institution we have to keep all tests to show various competencies have been met. We also recollect additional assignments as well.

When possible, I like to have multiple versions of a test. What I do is assign each group of students a specific letter designation, and use the tests from this letter designation in my files for their exams.
I never discuss this with the students, and they don't "know" that I have multiple tests.
Our college also requires that all exams be turned in at the end of the class to be used by accreditation for demonstration of learning.
I have found that although I might teach the same class over and over again, the editorial houses for the books have ways for instructors to change assignments and have different assignments for different modules.
I never have the same assignments in following modules.
