An online environment we are reduced to turnitin.com which works quite well. Even if a student opts to use some of another students work posted at the discussion board, we will be notified of the origin of the source.
Having taught business law/ethics, now mostly Grad school it is surprising to find out how many incidents of plagiarism I do encounter and the excuses are pretty horrible.

I also make it a practice to not be doing anything else while monitor testing, such as grading papers, reading, etc. You need to give test takers your undivided attention.

A well prepared student does not cheat. As an instructor it is my responsibility to give the student tools that can be utilized during test taking that will reduce anxiety.

There are many ways that an instructor can reduce cheating in class. The first way to do this is to make sure everyone know the punishment for cheating, and that you will not be afraid to enforce it. Having severe consequences will keep alot of students from cheating. The second way is to make sure that all uneccesary items are off of the desk or table.
Like many of my collegues, I use different versions of the exam and set my self in the back of the room. I caught one of my students cheating by writing down the content of the test as she was taking it. I took the paper and met with her after the test. Throughout the discussion, I discovered that she had a very difficult time with the terminology, not the content. It changed the way that I taught and also allowed her to see that I listened to her and modified my teaching style. She took the other version of the exam in my office to avoid cheating.
I have students sit "one away" (a space in-between)) from each other. This has worked very well as the students that know the material feel protected and not pressured to share their answers. I also stand in the back of the room and watch.

You are right cheaters will find a way. They spend a lot of time thinking about creative ways to cheat. You think you have all the answers to cheating, and they discover a new and fancy way to cheat. We just have to do the best we can to keep cheating at a minimum.

Steven Plonk: I agree with your style. Many students, for whatever reason, think cheating is the only way they can be successful. I work very hard to convince them they have to put in the work to learn, that I had to work as hard as they do, and that if it is interesting to them their goal can be achieved without cheating. Then it won't seem like work.
Being aware of what your students are doing is important. Unfortunately, cheaters will still find a way to cheat. Catching them can be difficult if you're not experienced in every possible method of cheating. Some are more obvious than others, but all have the same effect - lessening the learning experience for that student and possibly others. You need to be aware of every possibloe way it can be done to control it effectively.
Quizzes on Blackboard mix up the order of the questions and the order of the answers. This makes it much more difficult to cheat.

I like standing in front of the class which allows me to keep close eye contact on all students while testing. I have actually stood beside students which in which I believed were attempting to cheat. This has been a method as well as having three versions of each exam.
