Our catalog has almost everything there is to know about our programs.
The three items that I recall in regards to specific disclosures made by my institution are the cost of the program, the length of the program, and if required, information on athletic programs.

The disclosures that I know our institution gives is 1) The typical amounts charged for completing the program. 2) Disclosure of job placement rates and obviously 3) The jobs that the program prepares the student to enter.
I don't recall having inplace a disclosure for FERPA. However we do have to anually notify students of their rights as a student

Annual Security Report: The school publishes the policies and procedures for reporting crimes, as well as the types of crimes that have been committed on or near the campus, in the Campus Crime Report.
Loan Counseling: Each student who receives a student loan is required to attend an entrance and exit interview.The Financial Aid Office will notify each student when these interviews are scheduled.
Drug and Alcohol Abuse Prevention Information: The administration of Concorde maintains a list of hospital and community agencies available to assist employees and students seeking alcohol and drug counseling and treatment.
