What role does your Student Services Director perform? Is he/she focused on support services or social activities? How often do advisor make referrals to this person?
How do instructors share information about problem students? Do you have regular team or departmental meetings? Do you share written notes about conversations with the student? While I agree that it's appropriate for admissions representatives to remain involved with their students as they progress through their course of study, I'm not sure they have the skill set to be the go to person in the event a student has problems. However, it would be appropriate for them to be another resource in helping the student succeed.

Weekly newsletters are a great idea, but I would imagine could be burdensome. Does each faculty member send out the same information? Is there a template with school happenings and job postings provided to the instructors? If so, who prepares that common information?
Virtually every institution claims to have an open door policy, but students don't often use it. What's the experience at your school? Do your students use the open door policy and if so, what type of issues do they raise and with whom?
What's the level of participation in these volunteer events? The benefits are obvious to most of us, but what happens with the students who are unwilling or unable to participate?

Is there a fix for this? Who should take the lead in developing a better system and how will it be implemented?
How does a student end up on this list? What level of service do students who aren't in need of assistance receive?
How much variation is there in student schedules? How many students will a program director be responsible for?

Do the Registrar, Exec Chef and President meet weekly with every faculty member? How long do these meetings last? Who's responsible for completing the action plan? Has this been effective in improving retention?

What's the difference between an academic termination and probation? Are there situations where a student's appeal could be denied? How do the faculty feel about terminating a student for lack of academic progress only to see them back in their classroom after appeal?
