I understand your take on exams, but how realistic do you think this really is? Most exams that are taken in an academic setting or nonacademic setting are not group or open book exams.

I know it is hard to believe, but I have caught students cheating on open book exams. No matter how we plan ahead and how careful we are, there will be that oddball student who will attempt to cheat. Sometimes it seems it takes more time to prepare the cheat sheet than to study.
Once a student has been caught cheating, they receive written warning and then are required to sit close to the instructor or teaching assistant during tests. The student is rewarded for good scores without cheating
Do not give the students any opportunity to cheat. I have found that in my classes, the best way to keep this under control is to let them work on exams together and/or open book. I have found that in the subject that I am teaching there is no need to memorize the text in order for the students to pass. In my class they are there to learn, not memorize. I encourage being able to know where to find the answers, therefore, I do not have to worry about students cheating.
If I had a situation where I thought the students might cheat, I would make sure to make my presence known. I would most likely walk around the room or spread them out where they could not see other people's papers. I would also consider passing out different tests so could not copy the answers directly.

It is an excellent practice to make sure the desk is fully clear, including cell phones and water bottles. Cell phones and water bottles have been used as mechanisms to cheat with.

I try to keep it well monitored and make sure all desks are cleared prior to beginning the test.
By ensuring that there is enough space in between students as they are taking the test. Keep moving around the class to show that you are watching.
If space permits, seat students apart. My students are used to me moving students around the room to other seats prior to exams. This worked well on one occasion, when I placed one student who I suspected of cheating at the front of the class. It seemed like a normal thing to do rather than drawing attention to that particular student. If space is limited, having Test A and Test B is useful.

This can really throw a monkey wrench into their plans. They no longer have that particular person to cheat off of or to let cheat off of them.

I agree, having several form of the tests will discourage the students to cheat.

You do not have to hover to monitor an exam, but you should not being doing anything else while monitoring an exam, i.e., grading papers, reading, writing, etc. The students need to be watched!

This can be particularly difficult in the online environment. I am mindful of putting time limits on quizzes and managing release dates of unit content. I also review papers against other sources. Unfortunately, online there is much "opportunity" as compared with the classroom.
There are students that become friends in class and allow their friends to sheat off of them by positioning their exams or answer keys close to the cheating friends. There are also stuedents who don't care who they sit next to an will stiil look over. I normally implement two things, assigned and spaced out seating during an exam.
We require that all personal items be left at the front of the classroom. I usually stand at the back of the class to monitor during an exam.
