I understand exactly what you are saying about writing papers. This type of dishonesty can happen in a traditional classroom setting as well because we give homework. The homework paper can be flawless, and the classwork papers are awful.....you don't have to be a rocket scientist to figure this out, but as you have stated, it is very hard to prove.

I spread my class out all over my room. I have them sit at the x-ray tables and the classroom tables. That way no one is sitting next to someone. I also give out different tests with the same questions but different order of the questions.

To help avoid this is to help students understand the material more, make them feel more relaxed, give out different test each term so no one is the same, also with the test have different style questioning, multi choose, essay style questions.
We change out tests every two to three modules - this helps out tremendously....however our tests are all electronic therefore this reduces chances of them leaking out.
I teach online courses, so trying to determine if a student is having someone else do there work is almost impossible. This is where you have to be very trusting in your students. Believe it or not, you can tell just by their writings if they wrote it or not. One assignment could be written real well, while the other assignment could be written with several grammar and spelling errors showing you that they may not have written one of them. This is very hard to prove. The cheating that I encounter is plagarism. We use Turn-it-in to determine if the student has plagarized a resource. This is the best way to determine if a student has cheated in an online course.

It does appear that if you have a relationship with students they are less likely to cheat, however I have experienced the other end of the spectrum as well.

Make eye contact with the students. Get to know the students. Really listen to them. Create a relationship with them so that they are less apt to cheat.
I do not utilize the types of tests whereby students use True/False, or Multiple Choice. Those are easy to cheat off of. I utilize exercises where I ask students to write outlines for papers, bibliographies, evaluate sources, etc. Their answers will be wordy, so this limits the opportunities students have in cheating. Besides, the tables and chairs used in our classrooms are very spread out, so it would be difficult to look at someone else's paper.
We currently use netbooks in conjuction with an online testing program so the opportunities for cheating on exams persay can be controlled by be mobile in the classroom. Constant contact with the students through proximity minimizes the potential.

Some ways that you can reduce the opportunity for cheating in your class include good supervision, using multiple quizzes with different questions, and basically not provide the opportunity for cheating by being vigilant and watchful.
When I have a large number of students in my clas and am unable to keep enough distance between them, I make different versions of the tests. I also remind the students that they want to leave school with the knowledge to be successful in the work field.
Start by setting guidelines with full coverage of the class expectations. Giving student encouragement and a feeling of responsiblity for their own career choice.
Cheating is a conduct policy. Trust is something that we all earn and if student did do this action, that trust is broken.
Ways in which cheating can be avoided is by making different sets of exams, chair arrangement, teacher strict supervision, and explaining the consequence of such actions.
