We have evaluations at the end of each cours so the students can respond to managment if ther expectations are not being met.
our school also uses student surveys. it's used for student comments, good or bad. i have found them helpfull to me on several occations. but i don't think there realy helpfull to id students with problems.

Our institution uses a student survey at the end of each three week phase. The students are able to give feedback on Instructor performance as well as student services, financial aid, employment services etc. If the student chooses to put thier name on the survey, an education manager well speak with them personally. All instructors receive a copy of the survey so we can see how the students perceive us, and we can adjust accordingly.

They clearly explain in the admissions process the goals of the school and the skills that will be taught. While the students are in school they fill out survey's at the end of each phase, not only about how well the instructor performed, but if there expectations for the course were met.
Hi Loren, We have a student evaluation at the end of each course that is totally blind. the student is assured that unless they want discuss what is written it is held in complete confidence. Not only do the managers see the results and coments but all are sent to the home office for review. Any trends that are spotted are then worked on as an initiative
If people are asked to provide feedback and then nothing happens, it won't be long until they don't bother and the exercise becomes a waste of time, energy and good will.
How do you assure the students that they won't be punished for speaking out? Students tend to be a wary lot and question why they should take the risk without a clearly identified benefit for them.
At our institution student are given a survey after each class. This survey questions their satisfaction or dis-satisfaction with the class the instructor and the material in the class. The students have an opportunity to comment on these areas with out fear of being punished for speaking out. The comments and surveys are taken seriously and help develop an understanding of our strengths and weaknesses.
I can only answer that question based on what my experience has been, and that would be no. There may be some things that have changed as a direct result of this feedback, but if so, I am not aware of it.

We do the best we can at showing students the different options they have when approching a problem. We also try to form a bond on a personal level, giving the student a name and a face, makes easier to spot when a student is having trouble, like feeling home sick or lonely

Good question. Our institution does an instructor evaluation after each course where the students are provided an opportunity to analyze the course they just took and provide feedback - from the cuuriculum right up to what kind of job they felt that the instructor did.
First by setting clear expectations, what to expect from a specific course.
Second by evaluations for each course, and really following up with students.
The survey has 12 questions and students individually rate on a survey form. Responses vary but if the students have a good or bad experience they let us know. Students are very candid and have the opportunity to put their student number and ask for person follow-up.
Each phase is different. Each course is trended so we have a baseline and monitor progress.
The return rate is fairly high but we the majority just mark good or excellent without much thought. Students who've had a very bad or good experience provide useful data. Another factor is how the instructor briefs the student before the survey. Student data is trended for instruction, curricula, tools/training aids, and external resources and the data is used in our instructor development programs.
